Jefferson Community Health & Life is happy to make available to our patients and customers copies of their medical records which they may need. It is important that we establish the identity of the person requesting any medical records, as we are not authorized to give copies of medical records to anyone other than patients without expressed consent or proper paperwork establishing Power of Attorney, guardianship, or other appropriate relationship. In some cases, there may be a charge for copies of medical records.

You may also receive access to certain medical information from Jefferson Community Health & Life (primarily health center admission/dismissal information and health center lab results) through our Patient Portal. You can receive access to the Patient Portal by contacting us at 402.729.3351 and requesting an invitation for the Patient Portal. You will be asked to provide your name, the email address and/or cell number you used to sign up for My Wellness Dashboard, and your date of birth.
