Website

Jefferson Community Health & Life collects no personally identifying information about individuals except when specifically and knowingly provided by such individuals. Through this website, Jefferson Community Health & Life may place a “cookie” in the browser files of a user’s computer. A cookie is a small piece of text sent to your browser by a website you visit to help the website remember information about your visit, such as your preferred language and other settings. The cookie itself does not contain any personally identifying information, although it will enable us to manage banner advertisements presented to the user. This information will not be sold or rented to third parties.

We use the aggregated information we collect for the following purposes:

  • To improve the content and design of our website
  • To enable our audit bureau to verify our claims of traffic to the website

All email mailing lists provide the names and addresses of people who voluntarily provided that information to us. We collect those email addresses for exclusive use, including but not limited to promoting our products and services, conducting surveys, and consulting with website users. If you do not wish to receive an email from us in the future, contact us and we will remove you from the list.

SMS/MMS and Mobile Messaging Policy

We collect mobile phone numbers to communicate with patients via SMS and/or RCS text messages for purposes such as appointment reminders, billing notifications, and care coordination.

Your privacy is a priority. Your mobile number will not be sold or shared with third parties or affiliates for marketing or promotional purposes. We will not use your number for unrelated marketing without your express written consent.

Jefferson Community Health and Life uses text messaging to communicate with patients for purposes such as appointment reminders, billing notifications, and care coordination. You are not required to opt in as a condition of receiving care. Participation is voluntary. However, opting out may prevent us from sending you timely updates regarding your care.

Message frequency may vary.

Message and data rates may apply.

Carriers are not liable for delayed or undelivered messages.

Text messages sent via SMS and RCS channels are not fully secure and may not be HIPAA-compliant; however, we take reasonable precautions to safeguard your privacy by restricting the content of these messages to non-sensitive notifications. Any messages containing detailed Protected Health Information (PHI) will be sent via a separate, secure messaging channel.

You may opt out of receiving text messages at any time by replying “STOP.” You will receive a final confirmation text to verify you have opted out. No further messages will be sent. If you would like to rejoin, you can authorize us to restart by texting “START.”
