Cardiac Rehabilitation

Heart disease can be a life-changing diagnosis. Jefferson Community Health & Life provides an outpatient Phase 2 cardiac rehabilitation program designed to improve cardiovascular health after a cardiac event or procedure.

If you have had recent heart surgery, including CABG (coronary artery bypass surgery) or other cardiac procedures, or if you’ve had angioplasty with stenting (a stent placement), you may be eligible for cardiac rehab. Patients with heart failure may also qualify. Medicare and most commercial insurances cover cardiac rehabilitation. A physician’s order is required to attend.

Cardiac rehabilitation helps control symptoms of heart disease and teaches patients to reduce cardiac risk factors such as weight, high blood pressure, high cholesterol and smoking. What patients learn and practice during your cardiac rehab program will carry over into their daily lives long after cardiac rehab classes are complete.

Cardiac rehabilitation is a monitored exercise program of aerobic and resistance training exercise. It also includes education to reduce your risk factors for heart problems. Cardiac rehab includes nutrition counseling and stress management techniques. Cardiac rehab helps you gain strength, endurance and confidence to return to your normal daily activities.

Pulmonary Rehabilitation

If you’re living with chronic lung conditions or ongoing breathing issues, pulmonary rehab can help you feel stronger and more in control of symptoms. Jefferson Community Health & Life offers pulmonary rehabilitation, an individually tailored program that combines breathing techniques, guided exercise, endurance building, and education to help you live the fullest life possible.

Pulmonary rehabilitation may help people managing COPD (chronic obstructive pulmonary disease), pulmonary hypertension, pulmonary fibrosis, and other respiratory conditions. Pulmonary rehabilitation is covered by Medicare and many commercial insurances. A physician’s order is required to attend.
