Health Information & Medical Records

Jefferson Community Health & Life is happy to make available to our patients and customers copies of their medical records which they may need. It is important that we establish the identity of the person requesting any medical records, as we are not authorized to give copies of medical records to anyone other than patients without expressed consent or proper paperwork establishing Power of Attorney, guardianship, or other appropriate relationship. In some cases, there may be a charge for copies of medical records.

You may also receive access to your medical information from Jefferson Community Health & Life (such as hospital admission/dismissal information, lab results, radiology results, upcoming appointments, and provider visit notes) through our Patient Portal. The portal is free. You can self-enroll in the patient portal by going to the portal link at https://jchealthandlife.iqhealth.com/self-enroll/

You can also receive access to the Patient Portal by contacting us at 402.729.3351 and requesting an invitation for the Patient Portal. You will be asked to provide information and will receive an email link to access the Patient Portal.

You can access the portal through the HealtheLife app on your mobile device. In your app store, search for HealtheLife, and select Jefferson Community Health & Life as your facility of choice.
