Mau Named Quality and Population Health Manager
Jenn Mau MSN, RN, CRHCP, has been named Quality and Population Health Manager at JCH&L Fairbury Clinic. In her new role Mau will lead efforts related to Accountable Care Organization (ACO) initiatives, population health management, value-based care programs and quality improvement activities. Jenn will also provide leadership and oversight for Registered Nurses (RNs), Licensed Practical Nurses (LPNs), and other assigned clinical personnel engaged in care coordination, chronic care management, transitional care management, population health, quality initiatives, and other case management functions.
JCH&L Fairbury clinic continues to expand its focus on care management and helping patients achieve their best possible health outcomes through a Remote Patient Monitoring grant through the Rural Health Transformation Program (RHTP). This federal initiative, established by Congress under the One Big Beautiful Bill Act, is administered by the Centers for Medicare & Medicaid Services (CMS). In Nebraska, grant funding is awarded to the state and distributed through the Department of Health and Human Services. In addition, the clinic will be expanding its care management services to enhance the quality of care we provide and offer an even greater level of support to our patients between office visits.
Mau has contributed in multiple areas of JCH&L for 22 years, including roles in Gardenside Long-Term Care and Acute Nursing, as well as Clinical Care Coordinator, before most recently serving as Fairbury Clinic Nurse Manager.
“We are excited to expand our focus on care management. We want to help patients live their healthiest lives through the Remote Patient Monitoring grant,” Mau said.
